How Does Low Dose Naltrexone (LDN) Work?

Low-dose naltrexone works by briefly blocking opioid receptors in the brain, which triggers your body to produce more endorphins—natural compounds that reduce pain and improve mood. Dosing and Administration

LDN dosing varies from person to person, depending on the condition being treated and individual response. Treatment typically starts with a low dose—traditionally between 1.5 and 4.5 mg taken at bedtime—to optimize the body's natural opioid receptor response.  I prefer to individualize the dose and start most on 0.1 mg and a few on 1 microgram.

Your healthcare provider will adjust your dose based on your response and any side effects you experience. Since standard naltrexone tablets contain much higher doses, LDN must be prepared by a compounding pharmacy. These pharmacies can create customized capsules or topical formulations tailored to your specific needs.

Regular follow-up appointments help track your progress and fine-tune your treatment plan.

Side Effects and Precautions

Most patients tolerate LDN well, though some experience mild side effects during the first few weeks, including:

  • Vivid dreams or sleep disturbances

  • Headaches

  • Mild nausea or stomach upset

These effects usually fade as your body adjusts. If side effects persist, your provider may adjust your dose.

Important: LDN can interact with opioid medications, so a complete medication review is essential before starting treatment. LDN isn't appropriate for everyone—regular check-ins with your healthcare provider ensure the therapy remains safe and effective for your situation.
